Case Manager

Hamilton Health Center is currently recruiting for a Case Manager to work under the guidelines of the Healthy Start program. This position assists with recruiting program participants and provides case management support services to preconception, prenatal and inter-conception women, infants, and children and their families.
Educational qualifications include a bachelor’s degree in human services with a minimum of two years in human services work required. At least one years’ experience as a case worker preferred. Must have knowledge of community resources and programs and have respect for contributions and needs of multiple disciplines. Must be able to establish and maintain nonjudgmental, respectful relationships with clients, peers and other service providers, and communicate respect and empathy for cultural and lifestyle differences. Must be able to establish and maintain harmonious, productive working relationships with Center’s management, physicians, clinicians, and general public. Able to demonstrate accurate, clear, and concise verbal and written communication, documenting confidentiality regulations as required, possessing basic computer skills. Chief Financial Officer

Hamilton Health Center a Federally Qualified Health Center (FQHC), has an excellent opportunity available on our Executive Leadership Team for a Chief Financial Officer. The CFO is responsible for providing strategic and fiscal leadership for Hamilton Health Center, by working with the executive team to establish long-range goals, strategies, plans and policies. Oversees all fiscal operations of the Center, including general accounting, payroll, accounts payable, fee for service billing and patient accounting, managed care claims processing, and grant administration. Must be able to demonstrate financial management knowledge of health care services, with experience in clinical or non-profit service delivery organizations preferred, with familiarity with third-party reimbursement systems and cost reporting. Customer Service Patient Registration - Call Center

Hamilton Health Center is currently recruiting for call center help in our Medical Registration Department. The purpose of this position is to schedule patient appointments, manage the Providers’ schedules in all medical departments according to set protocol of each department and direct all incoming calls of the PBX in a manner that will enhance corporate image and increase customer satisfaction.

Medical Assistant

Hamilton Health Center is currently recruiting an experienced Medical Assistant to help support our growing Clinical Departments. This position provides assistance to Providers with direct patient care in an ambulatory care clinic setting under the supervision of a team leader, including initial patient triage, registration, and administrative services, assists with direct patient care, and performs specialized medical procedures of a routine nature, as dictated by established clinical protocol.
Qualified candidates must be able to demonstrate knowledge of health care services, including understanding of MA role and have excellent medical terminology knowledge and basic data entry computer skills. Nutrician Assistant I

Hamilton Health Center is currently recruiting a Nutrition Assistant full time with focus on providing nutrition education and breastfeeding support to WIC eligible participants as well as referrals to other social service organizations. This position requires the ability to learn basic principles and practices of nutrition and breastfeeding. Must present the ability to learn and apply anthropometric and hematological measurements. Must have the ability to present educational information clearly, both verbally and in writing. Must work well with the public, both in person and on the phone and be comfortable dealing with diverse racial, ethnic and socio-economic populations. Nutritionist I

Hamilton Health Center is currently recruiting a Nutritionist I who will be responsible for determination of WIC eligibility and nutrition risk levels mandated by state and federal government guidelines. Must possess knowledge of educational and nutrition counseling methods, nutrition assessment, and anthropometric and hematological measurements. Good documentation, basic computer skills and QuickWIC Data System is required. Must have good organizational skills and be able to perform simple mathematical procedures, such as computing family income. Also must be a team player with strong organizational and communication skills, able to establish and maintain harmonious, productive working relationships with Center’s management and general public. This individual must possess a B.S. from an accredited four-year institution, with emphasis on one of the following: Food and Nutrition, Community Nutrition, Public Health Nutrition, Nutrition Education, Human Nutrition, Nutrition Science or equivalent. Position requires the ability to handle moderate physical activity, including handling of average-weight objects up to 30 pounds or standing, sitting and/or walking for more than four (4) hours per day. Prevention Education Specialist Baby Love/Healthy Start

This position provides educational group sessions on various topics, information disseminations, and other community activities to youth within Dauphin County to prevent at risk behaviors within the county and promote healthy choice outcomes.

Psychiatrist, Part Time (20 Hours)

Hamilton Health Center, a Federally Qualified Health Center (FQHC), serving residents in Central Pennsylvania, is currently recruiting for a Part Time Psychiatrist (20 hours per week) reporting to the Chief Medical Officer.

This position provides integrated Behavioral Health Services to Hamilton Health Center’s patient population, and builds and maintains collaborative relationships with other members of the care team, including physicians, medical assistants, staff from behavioral health partner organization. RN - HOPE Program

Hamilton Health Center is currently recruiting a RN for our HOPE Program (HIV Outreach and Patient Empowerment). This position will work closely with the Chief Clinical Manager and the Adult Medicine Lead Provider as a clinical support position responsible for the general administrative and clinical needs of the providers and staff of the HOPE Department, to assure efficient running of operations and support providers with nursing care as needed. Incumbent must have strong supervisory/management skills in a clinical and public health care environment, be able to demonstrate knowledge of health care services, including understanding of nursing role and have excellent medical terminology. Must be industrious, have good communication skills and have the ability to develop rapport with professional staff. Must have knowledge of special health care services such as drug and alcohol facilities. Must be able to communicate politely and courteously with people from different socio-economic and ethnic backgrounds and be comfortable working with individuals with HIV disease. Social Media Outreach Worker (SMOW)

Hamilton Health Center, Inc., a Federally Qualified Health Center (FQHC), servicing residents in Central Pennsylvania, is currently recruiting for a Social Media Outreach Worker (SMOW) for our HOPE Program, reporting to our Chief Clinical Manager. The HOPE program treats HIV-infected individuals largely consisting of underserved minority groups, mainly African American and Latino. Our current HIV outreach involves their existing clinic population, local area high schools, jails and prisons with a focus on the MSM population. Outreach efforts for the proposed project will focus on these two underserved groups living in the Harrisburg vicinity and Dauphin County.
Key responsibilities will include marketing our brand in the community to youth/young adults including but not limited to the distribution of pamphlets with instructions on how to join the OPT-In sites, placing posters, providing small group educational sessions, etc. in the following new and existing sites/forums
All Hamilton clinic sites including their walk-in STD clinic

  • John Harris High School, Steelton High (Hamilton already has existing relationships with these schools), and Middletown High School
  • Camp Hill Prison and Dauphin County Prison
  • Abraxas Youth and Family Services (youth 12-18 with substance abuse)
  • Local stores and barbershops in the AA community of Harrisburg
  • Other areas where the MSM population frequents


Qualifications:
Must be able to demonstrate knowledge of special health care services, with experience in clinical or non-profit service delivery organizations preferred.
