Healthy Communities, & the Intern- denominational Ministers Conference
have partnered to form Harrisburg's first Public Health & Faith
Partnership. The goal is to mobilize the faith, health, and
education community to reduce health disparities affecting racial and
ethnic minorities. Over the next year, Hamilton will select seven
churches to participate in the partnership. |

Women, Infants, and Children (WIC)
Program
-
A partnership with the Pennsylvania Department of
Health that provides preventive health, supplemental foods, and
nutrition services to pregnant, breastfeeding, and postpartum women,
infants, and children. During the 2002 calendar year,
Hamilton's WIC Program served an average of 65,820
clients.
Hamilton Hope
-
Hamilton's HOPE Program, a partnership with US
Department of Health and Human Services, Bureau of HIV/AIDS, is
one of only two such programs in Harrisburg that provides
comprehensive HIV/AIDS SERVICES. Three specialized experts who
have extensive experience in treating patients with HIV/AIDS lead
Hamilton's HOPE TEAM. Dr. Arthur Williams is Harrisburg's only African
American, Certified HIV/AIDS Medical Specialist.

CHRS Cares Program

Hamilton's CRHS Cares Program is a partnership with
the US Department of Health and Human Services, Penn State
University, and the Program for Female Offenders. This
community partnership addresses issues of substance abuse and HIV
prevention for women.
Harrisburg Housing Authority
-
In partnership with the Harrisburg Housing
Authority, Hamilton provides onsite medical care to elderly and
disabled populations who reside in their senior high-rise apartments.
Baby Love
-
Under a contract with
the Dauphin County Drug & Alcohol Commission, Hamilton has created
the "Baby Love" Program, which provides prenatal &
medical care, cessation counseling, and social services to pregnant
substance abusing women.

Harrisburg School District
Partnership
- Hamilton and the Harrisburg City School District have partnered to
create a School Based Health Center @ Foose School. This
partnership directly impacts the lives of children and families by
providing medical, dental, and WIC services in South Harrisburg.

Collaborative for a
Healthier Harrisburg

Hamilton has
partnered with the City of Harrisburg, Harrisburg School District,
and Pinnacle Health System to create the "Collaborative for a
Healthier Harrisburg," which consists of 30 organization who
have mobilized to eliminate health disparities affecting racial and
ethnic minorities in Harrisburg.
