with diabetes self care management. The program, sponsored by the PA
Department of Health is open to all citizens of Pennsylvania without regard to

American and Latino clients due to the increased prevalence of diabetes in this
population.

Patients receive a blood glucose monitor and are instructed on its use. In
addition, each class includes a home cooked meal appropriate for those with
diabetes which the clients help to prepare. The class is taught by a
registered dietian and a registered nurse diabetic educator. After
graduation, the nurse diabetic educator provides follow up monitoring and care
