Hamilton Health Center, Inc. Notice Regarding Security Incident
Hamilton Health Center, Inc. (“Hamilton”) is committed to maintaining the privacy and security of the information that it maintains. Beginning on August 18, 2020, Hamilton notified certain individuals that an email with patient information was inadvertently emailed to an unauthorized third party.
Specifically, on June 19, 2020, Hamilton learned that a spreadsheet containing certain patient information was inadvertently sent to an unauthorized third party through an email phishing event. Upon learning of this issue, Hamilton immediately commenced a prompt and thorough investigation and began working closely with external cybersecurity professionals. Hamilton discovered that the spreadsheet contained some patients’ full name, member ID, date of birth and one or more of the following: diagnosis, treatment, physical condition medications, dates of laboratory tests and/or examinations, and/or the name of the patient’s provider. This incident does not affect all patients of Hamilton and not all of these elements were included for those individuals whose information was included in the misdirected spreadsheet.
Hamilton has no evidence that any information was or will be used for any unintended purposes. Notified individuals have been provided with best practices to protect their information. It is also recommended that affected individuals review the explanation of benefits statements that they receive from their health insurance providers and follow up on any items not recognized.
